– Legal form
– Domicile
– Share capital and splitting
– Matter to be changed (director change, change of address, change of share capital, etc.)
– Orders (e.g. 1 legalized extract of the commercial register after the change)
– Delivery address for orders of an updated extract
– Invoice address

- Send the form to the relevant cantonal commercial register
